Esemény leírása / Narrative:

Bejelentés szerinti eseményleírás: A felszállás után a futók helyzetét jelző sárga tabló folyamatosan égett. A jobb hajtómű hidraulika szivattyú nyomásesését jelző figyelmeztető tabló jelzés kigyulladt. A #2 hidraulika rendszerből a hidraulika folyadék elfolyt. A végső egyenesen bejött a #1 és #2 leválasztó szelep figyelmeztető lámpajelzés. A fékszárrnyat csak 5 fokig lehetett kibocsátani. A leszállás után bejelzett az orrkerék kormányzás figyelmeztető tablójelzés. A repülőgép megállt a gurulóúton. A gép törzsén és a földön hidraulika folyadék nyomai voltak láthatók. 


 

Esemény leírása / Narrative:

HA-LQB performed take off from Budapest (LHBP). After T/O all Landing Gear Doors Lights illuminated (amber colour). #2 ENG HYD PUMP Caution light came on, and the #2 HYD SYS pressure lost. On Final Approach #1 and 2 ISO VLV Caution Lights came on. Landing Flaps can not extended more than 5. After landing NOSE STEERING Caution Lights came on.The aircraft turned back to LHBP and made a safe landing and stopped on taxiway. Hydraulic Fluid found on the fuselage and on ground.
The aircraft totaly lost the hydraulic fluid in the No. 2 hydraulic system across the PTU. After replace of the PTU it was sent for shop visit.  The shop report showed a heavy wear on the pump side. The port cap and cylinder barrel internal face indicate possible over speed condition. The day before the serious incident of the aircraft there was an entry into the Flight Log "#2 engine hyd. pump light on". The maintenance replaced the hydraulic pump on eng #2. During the rectification, metal chips contamination were found in the drain filters of the hydraulic system #2. The case of the drain filters also was blocked by metallic chips. The failed hydraulic pump was on shopvisit at Parker. The shop visit report confirmed that the EDP had serious inside demage. Suspecting that the metal chips originated from the failed hydraulic pump (EDP) contaminated the hydraulic system and was the root cause of the serious incident. The maintenance flushing over the pipe lines of the hydraulic system #2. During the following landing gear test they found leakage at the reservoir of hydaulic system #2, replaced. After severel bleed of the the #2 hydraulic system there was a succesful test of the landing gears and a test flight performed. The aircraft returned to service.
